There is a new feature added to VitalSigns 8.6.4 which allows administrators to get alerts if a specific message in the outbound queue is older than a certain threshold, for example if the message has been sitting there for 15 minutes without being routed.  This feature was added for a customer that sends alerts to their customers, and they occasionally found that some messages would get stuck, even while others were fine.   Apparently it takes a while before Domino declares a message to be “dead” and this time frame was too long for this customer.

To enable this feature, select Preferences, Domino.
